Services Offered

IML Provides Support to people receiving services under the ID/DD medicaid wavier.

We provide day to day support of each person placed in a sponsored Residential home licensed under IT'S MY LIFE, LLC 

Implementing Individual Person Centered Plans, Community Living,

Medication Management, Nutritional Supports, Domestic Supports, Financial Supports, Leisure Supports, Transportation to and from all Medical Appointments, Health Club Center, Community Activities, etc.   

Sponsored Residential 

A residential service that consists of skill-building, routine supports, general supports, and safety supports that are provided in family setting homes providing supports under the supervision of a It's My Life that enable people receiving services to acquire, retain, or improve the self-help, socialization, and adaptive skills necessary to reside successfully in home and community settings.

 Non- Centered Base & Program Health Club

Non-Centered Base: 1 to 3 ratio that supports and fosters a person abilities to acquire, retain or improve skills necessary to build positive social behavior, interpersonal competence, greater independence, employability, and personal choices necessary to access typical activities and functions of community life such as those chosen by the general population. 

Health Club Setting: services provided to help acquire, retain, or improve skills of healthy life style , socialization, community integration, fitness, and adaptation via opportunities for peer interactions, community integration, and enhancement of social networks and a healthy life style
